Day of Discharge

A hospital discharge is a two-part process: "Part 1: Planning for Discharge" and "Part 2: Day of Discharge" tasks. Download our Discharge Checklist in English or Spanish below.

Part 1: Planning for Discharge

Part 2: Day of Discharge

Our goal is to discharge you by 11 AM on your day of discharge , so plan to leave before noon, and discuss the items on the checklist below with your doctor, nurse, and/or social worker throughout your hospital stay.

The following list is also in a downloadable Discharge Checklist that you can fill out and bring with you to the hospital (see bottom of page for English/Spanish links):

  • You or your caregiver know which companies will be providing your home care and have their contact number. (OR) You or your caregiver know the name, address, and contact number of the skilled nursing facility you are transferring to.
  • You or your caregiver have received and understand written discharge instructions.
  • Make sure you or your caregiver have all necessary paperwork for your follow-up care physician appointments.
  • You know which pharmacy your prescriptions have been sent to by your doctor.
  • You or your caregiver have your house keys, or someone is home to meet you.
  • Before you leave, make sure you and/or your caregiver check your room and bathroom for all personal items.

If your ride home does not arrive when you are ready to discharge, you may be asked to wait in the Main Hospital Lobby. You will be asked to call them to let them know they can check in with Security and pick you up near the Hospital Main Entrance.
