We maintain medical records to better serve you and your health care providers, and to meet certain legal and regulatory requirements. You - or any parties legally permitted to do so on your behalf - may request access to your protected health information any time, provided that all proper authorization requirements are met.

Medical records can be accessed through the Patient Portal or requests can be made through secured or regular email, phone call or by visiting the department between the hours of 8A and 4:30P, Monday through Friday.
