Accessing the Patient Portal

Is the Patient Portal free?

Yes, the Patient Portal is a free service provided to all of our patients.


How do I log into the Patient Portal?

If you are already signed up, to access the Patient Portal Login page, click here. If you would like to access the Portal using our Accessibility Login page, click here. To learn more about this graphics-free, "plain" Accessibility Portal option, see below. If you are not yet signed up, scroll down to learn how to register through Self Enrollment or Hospital Enrollment.


What if I need to access the Portal using an accessibility page?

Our accessibility Patient Portal is without graphical images, colors, and other design elements. This "plain" portal can be utilized via common software plug-ins such as screen readers for the visually impaired or translators into other languages. If you use these language tools, provided by services such as Google or Bing, your health information will be available in the language of your choice and can also be printed in that language.


What type of commitment am I making when I agree to the Terms and Conditions?

By agreeing to the Terms and Conditions, you consent to use the Patient Portal for non-urgent medical Portal related communication with the hospital. The information you may read on the Portal should not be considered complete and you should consult your physician for any medical inquiries. The Patient Portal is secure; however, if you download your Continuity of Care Document onto your computer, be aware that the document is no longer deemed secure by VPH. You can review the Terms & Conditions at any time by logging on to your Portal and selecting the link at the bottom of your Portal webpage.

Registering for the Patient Portal

How do I sign up for the Portal?

You may sign up for the Patient Portal using one of two ways:

Self Enrollment:

In order to self enroll, you need to have provided your email address during patient registration. If you did not, scroll down to Hospital Enrollment.

  1. Click here to access the Self Enrollment Request page.
  2. Enter your first and last name, date of birth, medical record number or social security number, and the same email address you provided at registration.
  3. You will receive an email from “My Health Patient Portal” with a link to create a username, password, and security questions.
  4. Once complete, you will be asked to log off and then log back on using your new username and password.
  5. Review and accept the user terms and agreement.
  6. You now have full access to your Patient Portal.

Hospital Enrollment:

Contact our Health Information Management (Medical Records) Department. Scroll to the bottom of this page for details. If the information you provide matches our records and your identity is confirmed, you will be granted access to your health information through the Patient Portal.


Why is it important that I provide an email address when being admitted to Valley Presbyterian Hospital?

If you provided your email address at the time of registration, you can self enroll in the Patient Portal from home. The Portal verifies your patient information by linking you to the email address you provided. If you did not provide an email address at the time of registration, you may still register for the Patient Portal by contacting the Health Information Management (Medical Records) Department at Valley Presbyterian Hospital. Scroll to the bottom of this page for contact details.


What if I lose my temporary username and password (or forget my username)?

You can call our Patient Portal hotline 818.902.7958 and a new temporary username and password will be emailed to your email address that we have on file within 48 hours. If you forget your chosen login password and click the “Forgot Password?” link, the Portal will send you an email with a link to reset your password. It will ask you to answer the security questions you created at the time of log in as identity verification.


What are the security questions that I am required to create used for?

Sharing Access and Content

Can my family have access to my Portal?

Yes, you can give family members, such as parents or healthcare proxies, access to your Portal. For more information regarding proxy access, call our Patient Portal hotline at 818.902.7958 to speak with a Health Information Management (Medical Records) representative.


If a parent has proxy access to their child's Patient Portal and Medical Record, how is the privacy of the child protected when he/she becomes an adolescent?

When the child turns 12, the parent or guardian will no longer have proxy access to the adolescent child’s health record and will not be granted access to the health record without the consent of the child. If the parent or guardian wishes to gain proxy access to the adolescent child’s health record, the parent or guardian must arrange a meeting with the Health Information Management (Medical Records) Department. The adolescent child must provide a signed authorization to the Health Information Management (Medical Records) Department in order to grant the parent or guardian access to the child's medical information through the Patient Portal proxy access.


What if I share my email address with someone else?

This is not advised, as account access should not be shared. Any messages generated by the Patient Portal to that email address will be seen by whoever has access to that email, without accountability. See section above on setting up proxy access.

Devices That Can Be Used

Does the Patient Portal work on my cell phone?

Yes, you can access our mobile friendly Patient Portal right from our website the same way as you would on a computer. You can even save the Portal link as a shortcut on your smartphone screen. Consult your phone’s help function for information on how to do this.


Does the Patient Portal work on every type of browser?

Yes, the Patient Portal is compatible with every type of browser. The Portal is compatible with the most current versions of Google Chrome and Mozilla Firefox, and Safari version 6 or later.


Is the Portal safe to use on different computers?

Yes, the Patient Portal is a safe and secure website that can be accessed from multiple computers and mobile devices.
