Patient & Family

The Relationship-Based Care Model is an operational framework that understands that healthcare is provided most effectively through relationships. The activities of care are organized around the needs and priorities of patients and their families, relationships among team members, and the nurse's relationship with themselves.

At Valley Presbyterian Hospital (VPH), our Nursing Patient Care Delivery System is reflective of Relationship-Based Care (RBC), wherein the care providers consistently keep the patient and their family the central focus. Healthcare teams exist for the care of patients. To that end, the patient and family are at the center of the interprofessional healthcare team. In order to meet patient needs and effectively coordinate care, a partnership is created among the patient, family, and healthcare team members. In this partnership, patients are treated with dignity and respect, informed of their rights and responsibilities, provided with information to facilitate their care and discharge, included in the planning of care and future options, and supported as the decision-makers of their care. Care providers endeavor to identify with each person's unique life situation and understand how they experience illness or injury.
