Patient Portal

As an added convenience to our patients, Valley Presbyterian Hospital has an interactive Patient Portal to securely access your health information online from any device and share the information with your healthcare team or designated caregivers. An Accessibility Patient Portal is also available for patients to use screen readers (for the visually impaired), language translators (for translating to other languages) or other accessibility tools.

We encourage all patients to enroll and actively participate in their healthcare. Once enrolled, you can view laboratory and radiology reports, medications, allergies, demographic information, visit history and discharge/aftercare information.

The best way to enroll is during your stay at Valley Presbyterian Hospital.

Signing up for the Patient Portal

You may sign up for the Patient Portal using one of two ways:

Self Enrollment:

In order to self enroll, you need to have provided your email address during patient registration. If you did not, scroll down to Hospital Enrollment.

  1. Click on the Patient Portal Self Enrollment link here.
  2. Enter your first and last name, date of birth, medical record number or social security number, and the same email address you provided at registration.
  3. You will receive an email from “My Health Patient Portal” with a link to create a username, password, and security questions.
  4. Once complete, you will be asked to log off and then log back on using your new username and password.
  5. Review and accept the user terms and agreement.
  6. You now have full access to your Patient Portal.

Hospital Enrollment:

  1. Contact the Health Information Management (Medical Records) Department helpline at 818.902.7958, available Monday - Friday from 8:00 AM - 4:30 PM. If the information you provide matches our records and your identity is confirmed, you will be granted access to your health information through the Patient Portal.

Language and accessibility

During enrollment, alternate links are provided which connect you to our Accessibility Patient Portal without graphical images, colors, and other design elements. This "plain" portal can be utilized via common software plug-ins such as screen readers for the visually impaired or translators into other languages. If you use these language tools, provided by services such as Google or Bing, your health information will be available in the language of your choice and can be printed in that language.

Using healthcare apps

The Valley Presbyterian Hospital (VPH) Patient Portal is an online tool that gives patients 24/7 access to parts of their medical records. This access also allows patients to connect their preferred healthcare app (typically on a mobile device) to view the medical record as another option to the web portal. After you have connected to the Patient Portal, you may also use your preferred healthcare app with the same username and password. If your app does not immediately connect to VPH, please notify a member of our team via the Patient Portal messaging function for assistance.
