As an added convenience to our patients, Valley Presbyterian Hospital has
introduced an interactive
Patient Portal to securely access your health information online from any device and
share the information with your health care team or designated caregivers. An
Accessibility Patient Portal is also available for patients to use screen readers (for
the visually impaired), language translators (for translating to other
languages) or other accessibility tools.
We encourage all patients to enroll and actively participate in their health
care. Once enrolled, you can view laboratory and radiology reports, medications,
allergies, demographic information, visit history and discharge/aftercare
The best way to enroll is during your stay at Valley Presbyterian Hospital.
What to expect:
If you wish to sign up for the Patient Portal at Valley Presbyterian Hospital,
you may do so by either asking your nurse or visiting the Medical Records
Department with your identification.
You may also
self enroll here. To safeguard your privacy, the following is required for self enrollment:
- You have been a patient at Valley Presbyterian Hospital and have provided
a valid email address during registration.
- You are able to use that email address to receive the first time login
- You enter your first and last name, as well as your date of birth, exactly
as on file in your patient records.
- You will need to know your Medical Record Number (from your discharge packet
or a recent bill), or the last four digits of your Social Security number.
When you successfully enter this information, you will receive an email
from "My Health Patient Portal" with a link to create your username,
password, and three personal security questions. Our system will use these
questions to securely identify you in the event that you forget your password.
Language and accessibility
During enrollment, alternate links are provided which connect you to our
Accessibility Patient Portal without graphical images, colors, and other design elements.
This "plain" portal can be utilized via common software plug-ins
such as screen readers for the visually impaired or translators into other
languages. If you use these language tools, provided by services such
as Google or Bing, your health information will be available in the language
of your choice and can be printed in that language.
