Each day, we make a promise to you and your family to provide excellent care and service. Our staff and physicians are committed to exceeding your expectations during your hospital stay, and we value your feedback.

We developed the Keeping Our Promise to You Program to encourage you and your loved ones to express your gratitude to our employees and physicians who demonstrate our Core Values and our Vision of "Excellence in all we do."

In our view, there is no higher compliment than being recognized by those we are privileged to serve. Individuals who are recognized through the program receive a copy of the completed form, a recognition certificate and will have their name displayed on public recognition boards located throughout the hospital.

The form may be directly submitted to a member of our nursing staff during your stay. You also may e-mail the form to HR.Department@valleypres.org , or submit via U.S. mail it to:
