VPH Partners with the Los Angeles Jewish Home for Care Transitions Program

VAN NUYS, Calif. —Valley Presbyterian Hospital (VPH) is pleased to announce a partnership with The Los Angeles Jewish Home to improve the coordination of care for Medicare beneficiaries and reduce hospital readmissions.

"We are pleased to work with The Los Angeles Jewish Home to encourage seniors to live healthy, independent lives and to reduce hospital readmissions in our community."

The Los Angeles Jewish Home is the largest single-source provider of senior housing in Los Angeles, and was selected as the lead San Fernando Valley agency for the Care Transitions Program. The program was created under the Affordable Care Act and is funded by the Center for Medicare and Medicaid Services (CMS).

The Care Transitions Program seeks to improve the transition of patients as they transfer from one health care setting to another. To facilitate this, transition coaches will be available to enrolled patients both at the bedside and during an in-home visit scheduled between 24-48 hours after discharge. The role of the coach will be to:

  • Help patients develop self-management skills so patients become more proactive participants in their medical care
  • Provide education on intervention techniques for chronic illness management
  • Provide access to transportation and mobile meal assistance
  • Coordinate and schedule follow-up physician appointments
  • Identify any "red flag" symptoms that suggest their condition is worsening and how to respond
  • Create a list of all medications to be reviewed by a pharmacist and to take to their physicians

It is estimated that over 1,300 beneficiaries will be served each year in the San Fernando Valley Care Transitions service area.
